EA COMMUNITY – PARENT/GUARDIAN CONSENT, LIABILITY WAIVER & TRANSPORTATION AGREEMENT Student

Permission to Participate:

For my child to participate in EA Community programs and activities, including educational discussions, recreational activities, organized sports, fitness activities, and supervised transportation related to EA Community programming.

2. Assumption of Risk – Physical Activities & Sports I understand that participation in sports and physical activities involves inherent risks, including but not limited to falls, collisions, strains, or other injuries. I acknowledge that my child is physically able to participate and voluntarily assume all risks associated with these activities.

3. Transportation Consent I authorize my child to be transported by EA Community staff or approved volunteers when necessary for program purposes. I understand transportation may occur in private or rented vehicles operated by licensed drivers. I accept the inherent risks associated with vehicle transportation and release EA Community from liability except in cases of gross negligence or intentional misconduct.

4. Release of Liability To the fullest extent permitted by law, I release and hold harmless EA Community, its directors, employees, volunteers, and affiliates from any claims arising from my child’s participation in activities or transportation, except in cases of gross negligence or intentional misconduct.

5. Medical Consent & Emergency Treatment In the event of illness or injury, I authorize EA Community staff or volunteers to obtain emergency medical care for my child if I cannot be reached. I agree to be financially responsible for any medical treatment.

6. Behavioral Expectations I understand my child must follow the EA Community Student Code of Conduct. If serious behavioral concerns arise, I may be contacted and required to pick up my child immediately.

7. Personal Property EA Community is not responsible for lost, stolen, or damaged personal items.

8. Photography & Media Release (Optional) ☐ Yes, I give permission for my child to be photographed or recorded for educational or promotional use. ☐ No, I do not give permission.

9. Pick-Up Authorization, please see registration form. Photo ID may be required.
